IMMUNIZATIONS OF STUDENTS
As required by state regulations (102 CMR 7.09 and
105 CMR 220.00), the Massachusetts Department of Public Health has
established the following requirements for attendance at school. A
Physician's certificate is the only acceptable evidence of proof of
immunization or disease.
PRIOR TO ENTERING SEVENTH GRADE:
A PHYSICAL EXAM
3 DOSES OF HEPATITIS B
3 DOSES OF POLIO
1 BOOSTER DOSE OF Td
2 DOSES OF MMR
1 DOSE OF VARICELLA*
(*2 doses if 13 years of age or older)
